PracticeQ Forms, Scheduling, and Payments

Certain appointment, intake, payment, and administrative forms may be hosted within the PracticeQ platform rather than directly on the public Wix website. Information entered into a PracticeQ-hosted form or payment workflow is processed through PracticeQ and is subject to the privacy, security, and contractual safeguards applicable to that platform. Master’s Hands Family Wellness may receive information necessary to schedule care, maintain records, process payments, or provide requested services.

Email, Text Messages, and Telephone Communications

By providing your email address or telephone number, you authorize Master’s Hands Family Wellness to contact you regarding your inquiry, appointment, account, or care, subject to applicable law and your communication preferences.

Standard email and text messaging may not always be fully secure. Do not use an unsecured communication method to send sensitive medical information unless the practice has instructed you to do so.

Telehealth Privacy and Security

Master’s Hands Family Wellness uses reasonable safeguards intended to protect communications and information associated with telehealth services. Patients are responsible for choosing a reasonably private location and protecting the privacy of their own device, network, and physical surroundings during a telehealth appointment.

Data Security and Retention

We use reasonable administrative, technical, and physical safeguards intended to protect information under our control against unauthorized access, alteration, disclosure, loss, or misuse. No website, electronic transmission, or storage system can be guaranteed to be completely secure.

We retain information for as long as reasonably necessary to fulfill the purpose for which it was collected, provide requested services, maintain required records, meet legal obligations, resolve disputes, protect the practice and its patients, and enforce applicable agreements and policies.

Children, Third-Party Services, and Your Choices

This website is intended for adults and for parents or legal guardians seeking information or services for a minor. 2. Cancellation & No-Show Policy

Effective Date: June 2026

Your appointment time is reserved exclusively for you. Timely notice allows us to offer that appointment to another patient who may need care.

Required Notice

You must cancel or reschedule at least 24 hours in advance. If you cancel with less than 24 hours’ notice or fail to attend your appointment, the credit card on file will be charged a $50.00 fee.

Late Arrival

Arriving more than 15 minutes late will be treated as a no-show. Depending on the remaining appointment time and clinical circumstances, the visit may need to be rescheduled.

Repeated Missed Appointments

Missing three scheduled appointments within a 12-month period may result in discharge from the practice. Any termination of care will be handled in accordance with applicable professional and legal requirements, including appropriate transition information when required.

Credit Card on File

Before the first scheduled appointment, patients are required to have a valid credit card on file, stored through the practice’s secure electronic health record and payment system.

The card on file may be charged for copayments, coinsurance, deductibles, late-cancellation or no-show fees, and outstanding balances more than 30 days old, consistent with the signed financial and practice policies.

Insurance and Payer Requirements

Cancellation and no-show fees are not billed to an insurance plan as a medical service. Any fee will be applied only when permitted by applicable law and the patient’s payer agreement.

Website Information Is Not Medical Advice

No Emergency Services

This website, its contact forms, email features, appointment tools, and patient messages are not intended for emergencies. Call 911 or go to the nearest emergency department when you believe you are experiencing a medical emergency.

No Automatic Provider-Patient Relationship

Viewing the website, submitting a contact form, or requesting an appointment does not by itself establish a provider-patient relationship. A clinical relationship begins only when the practice accepts the patient for care and an appropriate clinical encounter or legally recognized professional interaction occurs.

Appointment Requests and Telehealth Eligibility

An online request is not a confirmed appointment until the practice provides confirmation. Patients must provide accurate identity, contact, insurance, medical, and physical-location information.

Telehealth services are available only when the provider is legally authorized to care for a patient in the state where the patient is physically located at the time of the appointment. The provider may determine that an in-person evaluation, urgent care, emergency care, laboratory testing, imaging, or specialist care is more appropriate.

Prescriptions and Medical Decisions

No website statement or appointment payment guarantees a prescription, medication, laboratory order, referral, diagnosis, work excuse, prior authorization approval, refill, or requested treatment. Medical decisions remain subject to clinical evaluation, professional judgment, standards of care, and applicable law.

Payments and Insurance

Prices displayed on the website may change. Insurance participation does not guarantee that a particular service will be covered. Patients remain responsible for copayments, deductibles, coinsurance, noncovered services, and other amounts permitted by law and their insurance agreement.

Our Notice of Privacy Practices explains how protected health information may be used and disclosed, including treatment, payment, healthcare operations, patient rights, complaint procedures, breach responsibilities, and other uses and disclosures required by law.

Website access does not replace delivery of the notice. The practice must also provide the Notice of Privacy Practices through the appropriate patient intake or first-service process.

5. Right to Receive a Good Faith Estimate

Uninsured or self-pay patients may request a written estimate of expected charges for eligible non-emergency services.

A Good Faith Estimate is not a bill.

Patients may receive a Good Faith Estimate when scheduling eligible non-emergency services or when requesting one.

If a final bill is at least $400 more than the Good Faith Estimate, the patient may qualify for the federal patient-provider dispute resolution process.
